Experience

• Active Medical Staff Membership at the primary medical center in the community the practice is located in
• Other qualifications as listed in the PeaceHealth Lower Columbia Region Practitioner Employment Agreement

EDUCATION/LICENSURE/CERTIFICATIONS

Required:

• Graduate of Accredited Medical School with degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO).
• Completion of Accredited Residency.
• Board Certification in Specialty (must be obtained within three years of residency completion and keep! current).
• Unrestricted License to practice within the State of Washington.
• Possession of Federal DEA number without restriction.

WORK ENVIRONMENT

Medical center environment with frequent exposure to electricity, hazardous materials, hazardous waste, infectious waste, fumes or gases, radiation; occasional exposure to laser and constant exposure to blood borne pathogens and biological.

SENSORY/PHYSICAL/MENTAL REQUIREMENTS

Sensory: Frequent speaking; constant hearing; constant near and far vision; frequent depth

perception, peripheral vision, color accuracy, color discrimination and touch.

Physical: Frequent head/neck static position, rotation, flexion and extension. Frequent torso/back

static position, stooping, bending and twisting. Frequent simple grasp, firm grasp, fine

manipulation, pinch, finger dexterity, supination/pronation, wrist flexion and ability to

! operate hand controls. Occasional sitting. Constant standing and walkin! g. Frequent

crouching/squatting and occasional climbing stairs and ramps.

Mental: Must have the ability to learn and prioritize multiple tasks within the scope and guidelines of this position and its licensure requirements, many requiring extremely complex cognitive capabilities.
