Description: SEA MAR is at present recruited Mental Health Care (DL-L) Coordinator right now, this vacancy will be reside in Washington. Full time split position available in Monroe - 20 hours devoted to provide and coordinate mental health care and community referrals for clients enrolled in the DL (formerly the GA-U) benefit program by working closely with the primary care providers, mental health providers and ability to accurately present cases to a consulting Psychiatrist ; 20 hours devoted to Mental Health Therapist to provide assessments, referrals for psychiatric services, case management in coordination with other teams or parties involved with treatment; provide individual counseling, crisis stabilization, suicide risk assessment, treatment planning and participate in weekly multidisciplinary clinical reviews. Other Responsibilities: provides assistance to the Behavioral Health leadership in program development, policy and procedure development, compliance with WACS and RCWS; provides education and behavioral activation, monitors symptoms and response to medication and/or psychotherapy using structured instruments; and offers a brief course of counseling, as necessary. The care coordinator is responsible for timely and effective referrals to and communication with community-based resources, and for facilitating high levels of continuity of care.
Required Qualifications
MSW , MA, BSN with current RN license, Mental Health Professional, or Registered Counselor (Licensed Psychologist, LASW, LICSW, LMHC preferred) An understanding of chemical dependency treatment and an ability to coordinate mental health services with chemical dependency treatment providers. Washington State Department of Health Agency Affiliated Counselor Certificate required. Bilingual in English/Spanish preferred.
