We are currently hiring for a full time, permanent Case Manager in Forks, WA.
The case manager is an hourly employee responsible for providing support services to priority ment! al health clients, including the chronically mentally ill, seriously disturbed and acutely mentally ill.
The Case Manager may also provide some support services to alcoholism and/or drug clients, as assigned.
The Case Manager is under the supervision of the agency Director with clinical supervision provided by the Coordinator, of unit assigned by Outreach Director and/or Staff Psychiatrist.
ESSENTIAL JOB FUNCTIONS:
Must have current Registration Certificate for Counselors from Washington State
Department of Health.
Must be able to travel extensively within the catchment area and occasionally outside
the catchment area as needed.
Occasional long hours and constant mental application.
Will work in agency office, area meeting and classrooms, local jails, hospital settings,
Client residences and other areas of the community.
Must have valid Washington State driver’s license an! d be insurable.
Must be bondable.
Job Requir! ements
EDUCATION:
Bachelor’s degree in social science, social work or related area preferred.
Other specialized training may be substituted.
TRAINING AND EXPERIENCE:
A.
Two years direct experience in the treatment of mentally ill emotionally disturbed patients
and children.
B.
Must demonstrate knowledge of mental health case management in accordance with
WAC 388-865.
C.
Must demonstrate knowledge of community issues and dynamics effecting case
management services.
D.
Must demonstrate knowledge of location, range, and type of services outside of the
catchment area for the purpose of referral.
E.
Must demonstrate the ability to use good judgement and skills and be capable of
performing required responsibilities with minimum supervision.
F.
Must provide professional references which in! dicate good job attendance, job
performance, professionalism, reliability, ethical behavior consistent with established professional
standards.
G.
Must provide documentation of all relevant education, training and credentials required
for the position.
H.
Must be able to train and supervise volunteers.
I.
Must have current First Aid card.
