Location: Yakima Washington

Description: Central Washington Comprehensive Mental Health is hiring Outpatient Registered Nurse (RN) right now, this vacancy will be placed in Washington. Responsibilities include providing direct/indirect nursing services to outpatients who require or will benefit from nursing skills and interventions. This includes, but is not limited to:
Medication Administration : Conducts and documents ongoing evaluation of side effects, adverse reactions, medication non-compliance and conformance. Safely administers and documents injectable medications. Provides client,/family medication education, including purpose of medication, side effects, dosage, frequency and! duration. Provides client/family medication education, includ! ing purpose of medication, side effects, dosage, frequency and duration. Administers ordered methadone medication accurately and documents. Accurately inventories and audits methadone medication logs.
Symptom and Medication Response Monitoring : Conducts and documents ongoing assessment of client symptoms and their responses to prescribed medications. Provides support and education to encourage clients to take medications as prescribed. Promotes illness self-management skills to assist clients with medication adherence, identifying warning signs and symptoms, and symptom management skills. Monitors medication side effects and facilitates access to, and communication with, medical staff as needed.
Treatment Planning : Completes all required treatment plans following Comprehensive' s guidelines and policies for content, format and timeliness. Applies assessment, evaluation and reassessment data to the formulation of treatment plans. Supports recovery pri! nciples by incorporating the individual's and their family's (when applicable) strengths, barriers and needs into the plan components. Documents problem statements and treatment goals/objectives/discharge plan that are written in clear and understandable language and includes client's own voice. Formulates measurable, achievable goals and objectives within the scope of treatment provided in collaboration with the client. Describes clinical modalities/interventions that are responsible and provided according to applicable Comprehensive protocols. Reassess treatment effectiveness and adapts treatment plan as needed in collaboration with the individual client.
Progress Notes : Completes all required progress notes according to Comprehensive's guidelines, policies and procedures regarding format, content and timeliness. Language used in progress notes is respectful, clear, descriptive and client-centered. Progress notes accurately depict treatment modalities and inte! rventions provided within the clinician's scope of practice and accordi! ng to treatment plan. Progress notes accurately reflect and depict client status and progress per assessment and reassessment data, problem statements, and treatment plan goals and objectives.
Question, Persuade, Refer, Treat (QPRT): Applies QPRT principles in assessing risk and safety and treatment planning. Develops and documents appropriate safety plan.
Carries out medical orders as prescribed/directed by staff physicians in a concise and timely manner.
Refers clients to hospital/Designated Mental Health Professional if no less restrictive option is available.
Provides tardive dyskinesia screening bi-annually. Alerts psychiatrists to possible dyskinesia observed.
Provides educational information, protective devices as prescribed by psychiatrists concerning STD's and AIDS.
Establishes and maintains family support by providing information about disorders, medications, and treatment, either requested by team or client.
Notifies treatment t! eam of client medical non-compliance and/or missed appointments with nurses. Provides triage services to teams.
Schedule : Monday-Friday 7am-4pm or 10am-7pm or combination of both. Schedule may include weekends. Will participate in an on-call rotation after hours.
