Profile Create a Job Agent Update Job Agent Mental Health/ Substance Abuse Counselor - 45th St. Youth Clinic Tracking Code 340-260 Job Description The Co-Occurring Counselor (MH/CD) at the 45th St. Homeless Youth Clinic will assist with identifying, and managing patients with behavioral health problems within the primary care setting. The Co-Occurring Counselor (MH/CD) develops specific behavioral change goals and implements treatment with patients, in collaboration with medical and psychiatric staff at their site.
Work in partnership and joint accountability with other team members to achieve NeighborcareâÂÂs Mission, Guiding Principles and Goals. Position is Part-time, Evenings, 9 hours a week: Wednesday 5pm-10pm Thursday 5pm-10pm Assist medical providers to engage the patient in appropriate behavioral health services, either with Neighborcare Health or another agency. Work with patient, consulting psychiatrist, and PCP to refer patients to community mental he! alth centers when appropriate, and support patients during tra! nsition. Work with other Neighborcare staff to identify and refer patients with chemical dependency or abuse problems.
Assist PCPs and consulting psychiatrist in assessing the clinical status of patients and in correctly diagnosing mental disorders. Assist PCPs and consulting psychiatrist with patient follow-up regarding medications, side effects, effectiveness, and assist patient with education and support regarding medication issues. Work with consulting psychiatrist to troubleshoot patientâÂÂs medication problems and/or lack of effective response to treatment. Using evidence-based practices, provide brief consultation and therapy interventions with patients, in person or by telephone, focused on symptom management and/or healthy behavior changes to alleviate distress, build mental health, and prevent relapse.
Facilitate skill-building and psycho-educational classes or support groups, including creating curriculum for these classes. Attend Behavioral ! Health Consultation with credentialed supervisor twice a month (hours can count toward licensing requirements). Provide culturally competent and sensitive services to patients. Required Skills Provide high quality behavioral health services to patients.
Work with other Neighborcare staff to identify and refer patients with chemical dependency or abuse problems. Able to assist patient follow-up. Use evidence-based practices. Able to practice from a harm reduction approach.
Facilitate skill-building and psycho-educational classes or support groups. Provide culturally competent and sensitive services to patients. Required Experience MasterâÂÂs degree in Social Work, Counseling or related field, or PhD in Psychology. Washington State Associate license (MHCA, MFTA, SWAA or SWAIC) or full licensure as LICSW, LMFT, LMHC, LACSW, or Clinical Psychologist.
Mental health and substance abuse counseling and assessment experience highly preferred. Experience! working in a medical setting with medical providers preferred.
