Care Coordinator

The Care Coordinator functions as a core member of a collaborative care team that involves the patientâs primary care provider, a consulting psychiatrist and other mental health providers that m! ay be available in the primary care clinic. The Care Coordinator is responsible for delivering integrated mental health care in consultation with a primary care team, and coordinating referrals to clinically indicated services outside the primary care clinic (e.g. social services, mental health specialty care, and substance abuse treatment).
Essential Duties and Responsibilities
Provide mental health care screening, education and services to identified patients to promote improvement in medical and mental health.
Is responsible for facilitating program workflows and improvements at his/her site so as to achieve program quality and performance aims.
Coordinate care to those patients and families who may have difficulty in accessing medical benefits and community referrals.
Document services rendered in patient electronic records and/or other databases such as MHITS as required.
Monitor patients in person or by telephone f! or changes in clinical symptoms and treatment side effects or ! complications.
Provide brief counseling or psychotherapy using evidence-based techniques such as behavioral activation, motivational interviewing or other relevant skills.
Provide or facilitate outside referrals to evidence-based psychosocial treatments as clinically indicated.
Conduct regularly scheduled caseload consultation with a psychiatrist and communicate resulting treatment recommendations to the patientâs PCP or other care team members.
Facilitate patient engagement and follow-up in health care.
Track patient treatment, follow-up and clinical outcomes using the web-based MHITS care management tracking system. Document in person and telephone encounters in MHITS and use the system to identify and reengage patients who may be lost to follow-up.
Secondary Duties and Responsibilities
Attend staff meetings, in-service meetings, team meetings and participate in agency committees and task force activities as! required.
Participate in community outreach events and networking opportunities so as to enhance the programs success in working collaboratively in the Military, VA and other support service arenas.
Engage in public speaking and/or write educational materials.
Support reporting requirements per grant contracting requirements.
Requirements
Demonstrated ability to collaborate effectively in a team setting.
Comfort with the fast pace of primary care.
Ability to effectively communicate with primary medical care providers about mental health issues treatment.
Ability to effectively engage patients in a therapeutic relationship.
Experience with screening and assessment for common mental/substance use disorders.
Working knowledge of differential diagnosis of common mental/substance use disorders.
Working knowledge of evidence-base psychosocial treatments for common mental disorders.!
Working knowledge and experience with brief, structured couns! eling techniques (e.g. Motivational Interviewing, Behavioral Activation).
Basic knowledge of psychopharmacology for common mental disorders.
Education and/or Experience
Care Coordinators shall be qualified as a Mental Health Professional under WAC 388-865-0100, or
qualified as a Mental Health Care Provider and supervised by a Mental Health Professional or Masters in Social Work.
Masterâs degree in Social Work, Counseling, or related field; and one to three years related experience working with veterans, mothers and children, or other high risk groups; or equivalent combination of education and experience.
Preferred: Experience working with underserved, transient populations: particularly disabled single adults and either Military personnel and their families, or new moms and children.
Working with patients who have co-occurring mental health, substance abuse, and physical health problems.
Worki! ng knowledge of Disability Lifeline, SSI program requirements and DSHS eligibility program requirements.
Computer Skills
Demonstrated operating knowledge of computers. Intermediate level ability with Word, Excel, Outlook and other required software programs.
Licenses or Certificates
Licensed Mental Health Counselor or Mental Health Counselor Associate.
Licensed Marriage and Family Therapist or Marriage and Family Therapist Associate.
Licensed Social Worker or Social Worker Associate.
Certified Counselor or Counselor Associate.
