Western State Hospital (WSH) has an opportunity for a Psychiatric Social Worker to work collaboratively with a multitude of disciplines such as nursing, doctors, psychiatrists, psychologists and community members in the Psychiatric Treatment and Recovery Center (PTRC).

You will perform psychiatric social work, case management, and collaborative work with the legal system including creation of services linkages and discharge planning for our specialized patient population.

Some of what you'll do:

  • Work within a multidisciplinary team
  • Perform and develop discharge plans
  • Act as a liaison between the family, hospital and community in coordinating the discharge planning process
  • Advocates on behalf of the patient and works closely with home and community services and Department of Correction
  • Educates and counsels patients and their families about psychiatric illnesses, diagnoses and prognoses
  • Facilitate evidence based psychotherapy and psycho educational groups
  • Complete initial and on going documentation; psychosocial assessments, progress notes and formulation of evidence based treatment plans
  • Attend hospital case case consults and participate in legal hearings
  • Submit referral for guardianship and assist with durable powers of attorney
  • Work closely with the financial department for financial eligibility as it pertains to each patient

What we're looking for:
  • Facilitate psycho-educational or therapy based social work group
  • Provide individual and group interventions to patients based on their treatment goals
  • Work cooperatively with a multi-disciplinary treatment team to formulate treatment goals and assess patient progress toward patient recovery and discharge readiness

Who should apply?
Professionals who have a Master's degree in social work and two years of experience in counseling mentally or emotionally disturbed individuals

You must have an Agency Affiliated Counselor Registration
or an independent license is preferred

Degrees must be obtained from an accredited institution
