Behavioral Health Consultant
QUALIFICATIONS
Education/Training: Masterâs degree in Social Work, Counseling, or Psychology
Washington State Licensure: Licensed Clinical Social Worker, Licensed Mental Health Counselor, or Licensed Clinical Psychologist
Experience: A minimum of two years post-masterâs experience
Abilities:
- Ability to manage caseload of approximately 50-60 adult patients at any given time with a total of approximately 100-150 patients annually. Patient population is culturally, economically, and racially diverse. Many patients are underserved, transient and with clinical co-morbidities. Desire to treat patients in accordance with the mission and philosophy of Country Doctor Community Health Centers.
- Must have the ability to make quick and accurate d iagnosis of mental health and substance use issues and develop treatment plan based on diagnoses. Clinical skills in problem-solving and behavioral change strategies, including motivational interviewing and behavioral activation.
- Experience and expertise in working in collaborative or team-based models of care.
- Ability to monitor adherence to care plan, evaluate effectiveness, monitor patient progress in a timely manner and facilitate changes as needed.
- Good working knowledge of psychopharmacology.
- Ability to function independently and in a team, interfacing with patient, primary care providers, consulting psychiatrist and mental health specialists, as well as with administrative and support staff.
- Ability and willingness to work in a time-limited, structured, and solution-focused environment.
- Knowledge of medical co-morbidities.
- Ability to multi-task and prioritize competing requests effectively.
- Flexib ility to adapt to unforeseen needs or circumstances.
- Familiarity with community resources for specialty mental health, substance use, and support for social stressors.
- Excellent verbal and interpersonal skills.

POSITION SUMMARY: The Behavioral Health Consultant is part of patientâs primary care team and works in collaboration and partnership with the patient, patientâs family, primary care provider, nurse, psychiatric consultant, clinic specialty care teams, peer support specialist, community mental health specialist, and community resources in a team approach to guide patient in the self-management of their own health and well-being.
JOB RESPONSIBILITIES:
- Responsible for managing a panel of patients with identified mental, substance-use and/or medical co-morbidities that are sub-optimally controlled.
- Engages patients to move from passive to active member of their healthcare team.
- Works collaboratively with multiple teams to implement evidence-based care to adult primary care patients.
- Proactively monitors population/caseload to meet treatment goals and guidelines.
- Follows guidelines and shapes decision-making based on practice and/or treatment teamâs agreed upon goals.
- Provide referrals for psychiatric/substance use services and care coordination with other teams involved in patientâs treatment and care.
- Advocate for patients while maintaining a patient-centered approach that allows the patientâs desires around behavioral change to inform the plan of care.
