Summary

The Behavioral Health Specialist will perform Mental Health and/or Substance Use Disorder outpatient counseling at Learning Center North within Shoreline Community College. This position will work from an integrated behavioral health modal, providing a broad spectrum of culturally appropriate services, including completing behavioral health assessments with standardized behavioral and health risk screenings, providing group and individual counseling, help with case management, and make referrals to students with behavioral health/academic needs. Additional indirect services will include providing consultation services to the primary care providers and the school staff, consulting with psychiatry, making psychoeducational presentations to staff and students about behavioral health, and providing liaison services between school staff and primary care providers. This position is subject to meeting performance expectations and grant continuation.

Duties and Responsibilities

  • Provide behavioral health outpatient counseling (Mental Health, Substance Use Disorder, Co-Occurring) to students age 16-24.
  • Assist with screening, identification and management of patients with behavioral health/academic difficulties, victims of abuse, neglect, domestic violence, rape, trauma, etc.
  • Provide assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, academic progress, and/or life situation.
  • Obtain and study medical, psychological, social, and family histories, by interviewing individuals, couples, or families and by reviewing records.
  • Provide psycho education to patients and families around issues related to adaptation to the patient’s illness, academic difficulties, and/or life situation.
  • Develop and implement treatment plans, specifying type, frequency, intensity, and duration of therapy.
  • Evaluate the effectiveness of treatments, and the accuracy and completeness of diagnoses, and then modify plans and diagnoses as necessary.
  • Consult / collaborate / coordinate with the health care team, psychiatrist, school personal, outside professionals, and the patient / family in the development and implementation of treatment plans whenever required or clinically relevant.
  • Monitor clients in person or by telephone for changes in clinical symptoms and treatment side effects or complications.
  • Perform case management and provide social services when appropriate.
  • Provide brief interventions using evidence-based techniques such as behavioral activation, motivational interviewing, or other relevant skills.
  • Provide or facilitate in clinic or outside referrals to evidence-based psychosocial treatments such as the Cognitive Behavioral Therapy, Problem Solving Treatment, Stress Management Training, Dialectical Behavioral Therapy, and Behavioral Activation as needed.
  • Conduct regularly scheduled caseload consultation with Learning Center North Case Managers
  • Facilitate patient engagement and follow-up in health care
  • Track patient treatment, follow-up, and clinical outcomes in the medical record and/or other databases as required. Document in person and telephone encounters in medical record and other databases. Use database and medical record system to identify and reengage patients who may be lost to follow-up.
  • Document services rendered in patient electronic records and/or other databases as required.
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCP and other behavioral health providers. These may include changes in medications or psychosocial treatment or appropriate referrals for additional services.
  • Facilitate referrals for clinically indicated services outside behavioral health (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, in-patient substance abuse treatment).
  • Facilitate program workflow and improvements at his/her site so as to achieve program quality and performance aims.
  • Participate and represent behavioral health perspective in multidisciplinary meetings and trainings.
  • Support reporting requirements per grant contracting requirements.
  • Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications.
  • May provide psychological services to patients and staff in the event of disasters and emergencies.
  • May provide consultation/supervision to other behavioral health professionals.

Job Requirements

  • Education â€" Master’s degree in social work, psychology, counseling, or related field; and be qualified as a Mental Health Professional under WAC 388-865-0150. Current and active credential as a Licensed Independent Social Worker or Licensed Mental Health Counselor with the Washington State Department of Health.
  • Experience â€" Three (1) year experience working with youth of color and/or other high risk groups.
