Duties:

Duties:
The incumbent serves as a Social Worker Program Coordinator for the Admissions function of the Mental Health Residential Rehabilitation Program (MR RRTP) Domiciliary and the Compensated Work Therapy/Transitional Residence (CWT-TR) program at the American Lake Division(s) of the VA Puget Sound Health Care System. The incumbent's primary duties involve providing clinical care in a residential rehabilitative setting to eligible Veterans who have a wide range of medical, psychiatric, social and vocational needs. The incumbent provides management and coordination of consults as well as screening, timely scheduling and disposition of Veterans with complex needs referred for admission to the MH RRTP and CWT-TR Programs. The incumbent serves as the primary point of contact for referring providers and for Veterans referred to the programs which may require the incumbent to interface with multiple treatment teams. The incumbent's programmat! ic responsibilities include management and communication of ad! mission-related issues, compilation and maintenance of admission-related data, and responsibility for utilizing this data, as well as VISN and national policies and guidelines, to inform local policy and practice.

Administrative Responsibilities (1) Responsible for the administrative oversight and /or coordination for the MH-RRTP and CWT-TR programs admission process. Specific responsibilities include: Manage database to track Veterans referred to and/or admitted to the MH-RRTP and CWT-TR program. (2) Collaborate in the performance improvement process and utilize expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the MH-RRTP and CWT-TR programs, utilizing outcome measures to improve treatment services and access to care. (3) Participate in activities that obtain feedback from persons served and work to enhance services as needed. (4) Assists in the writing of policies, procedures and/or practice g! uidelines for the MH-RRTP and CWT-TR programs as necessary in coordination with the Team Leader and/or Program Director. (5) Assists the Supervisory Program Director in the design, development, and implementation of clinical assessment and/or intervention programs. (6) Adheres to operational policies and procedures for the treatment team, established by the affiliated Team Leader, Program Director, and/or Service Line Leader. (7) Assists the Team Leader and/or Program Director in the execution of ongoing program evaluation and data-driven quality improvement projects to monitor the effectiveness of the clinical program. (8) Facilitates liaisons with other health care programs in order to effectively coordinate services. (9) Complies with national and local performance measures and facilitates organizational success on these measures at the team level, (10) Complies with all Facility and Service Line policies and expectations concerning administrative practices, including bu! t not limited to leave authorization, responding to Suspense items, and! completing TMS educational requirements. (11) Attends and participates in administrative and informational meetings held by the Social Work Director/Deputy Director, Service Line Leader (or designee), Chief of Staff, and/or Facility Director. (12) Maintains current knowledge of, and practices in accordance with, Joint Commission requirements. (13) Protects printed and electronic files containing sensitive data in accordance with provisions of Federal privacy acts and confidentiality statutes. Follows regulations and instructions regarding release of sensitive or protected patient or employee information.
Clinical Responsibilities (1) Provides comprehensive mental health and psychosocial assessment and diagnostic services for Veterans with a broad range of mental health issues receiving care within the MH RRTP and CWT-TR programs, including services for patients with complex psychopathology and psychosocial needs requiring advanced diagnostic and intervention skills. (2! ) Demonstrates expertise in using and contemporary interview and psychometric assessment methods for mental disorders and/or behavioral health conditions. (3) Provides education to patients and family members regarding mental disorders and best practices approaches to treatment. (4) Provides evidence based psychotherapeutic interventions as indicated and appropriate for the specific job setting and duties of the social worker, which may include individual family and group psychotherapy and psychoeducation. (5) Provides consultation to health care providers about assessment findings and treatment plan for patients to ensure that care is coordinated, collaborative, and integrated within the Medical Center. (6) Develops, implements, and documents the mental health treatment plan for an assigned caseload of patients, as indicated and appropriate for the specific job setting and duties of the social worker. (7) Coordinates community-based services, including information and refe! rral for additional services from another programs other government com! munity agencies. (8) Possesses general knowledge of VeteransÂ' benefits and services relating to special programs, service connected compensation, and non-service connected pension. Facilitates referrals based upon VeteransÂ' needs and eligibility. (9) Independently collaborates in identifying and addressing ethical issues that impact patients and staff including initiating and participating in ethical consultations. Maintains complete Veteran confidentiality. Analyzes ethical/care issues, applying appropriate theoretical principles and ensures application of institutional policy. Provides considerate care, respecting Veteran/family and staff personal values and belief systems. (10) Develops working relationships with the community and VA providers to whom referrals ate most often made and becomes knowledgeable with what community and VA resources are available to Veterans (i.e., related to housing, work readiness, food, financial, independent living skills, leisure skills,! social, healthy living resources). (11) Refers Veterans to a wide variety of community and VA housing resources, maintains the relationships with those respective facilities/agencies, facilitates inter agency communication and care conferences as needed and provides discharge planning for Veterans that maintains a continuity of care. (12) Complies with all Service Line, documentation requirements, including standards for timely completion of CPRS patient contact notes, use of required documentation templates, clinical reminders, treatment plans, and risk assessment protocols. (13) Adheres to all Facility and Service Line policies and practices for delivering clinical care, including but not limited to risk assessment and reduction practices for potentially dangerous behavior (suicide and violence), timeliness of care and access standards, professional conduct requirements, and LWRS documentation convention.

Qualifications & Evaluations

BASIC REQUIREMENTS:
The following are requirements for appointment as a social worker in the Veterans Health Administration! (VHA):
a. Citizenship: Be a citizen of the United States. b. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
c. Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the masterÂ's degree level.
d. Physical Requirements: The physical demands of this position involve primarily walking, talking, use of the telephone, etc. Strenuous physical activity is not required although the incumbent is expected to withstand pressure and stress as part of their day-to-day activities. A physical examination prior to employment is required.
e. English Profici! ency: Social workers must be proficient in spoken and written English.
f. Selective Factor: One year of post MSW degree experience in the field of clinical care in a residential rehabilitative setting to eligible Veterans who have a wide range of medical, psychiatric, social and vocational needs.

GRADE REQUIREMENTS:GS-12 Social Worker:
Experience, Education and Licensure: Individuals assigned as a social worker for a specific program or as a program coordinator must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-masterÂ's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. The program ! coordinator manages the daily operation of the program, develop policie! s and procedures for program operation and prepare reports and statistics for facility, VISN, and national use. They also may be responsible for program's budget. PROOF OF AND COPY OF ADVANCED PRACTICE LICENSE MUST BE PROVIDED.

In addition, the candidate must demonstrate the professional KSAs in below.
1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness. Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority for services and provide specialized treatment services.
2. Advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family cou! nseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans with polytraumatic injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, etc.
3. Advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on empirical findings.
4. Ability to provide subject matter consultation to colleagues and students on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. Ability to teach and mentor staff and students in the specialty area of! practice and to provide supervision for licensure or for specialty cer! tifications.
5. Ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program.
