DUTIES:
The Suicide Prevention Case Manager will be assigned to the Mental Health Service Line at the American Lake VA Medical Center, Tacoma, WA

The principle role of the Suicide Prevention Case Manager is to coordinate and monitor services for veterans designated as "High Risk for Potential Suicide", and act as the transitional coordinator of care for these patients. The incumbents are responsible for coordinating the care of high risk veterans throughout the medical center, including CBOCÂ's and community agencies. The incumbents are expected to follow VACO guidelines with regards to management of patients at "High Risk for Potential Suicide." The functions of the case manager include,

but are not limited to:
- Collaboration with inpatient treatment providers to support appropriate discharge planning following recommendations for treatment of patients at high risk for suicide. - Assuring that all veterans assigned to the facility "High Risk for Potential Suicide" list are followed per VACO guidelines. - Contacting the veteranÂ's Principal Mental Health Provider to coordinate monitoring activities, tracking appointments and coordinating enhanced care as needed. - Conducts: routine suicide risk screens; comprehensive assessments and collaborative safety plans when clinically indicated; follow-up appointments for all veterans upon initiation on the "High Risk for Potential Suicide List" in compliance with VACO directives; follow-up with veterans who fail to attend appointments per local procedures; consultation with the Suicide Prevention Coordinator, a member of the Suicide Risk Reduction Committee or a MHS supervisor as needed to ensure the safety and welfare all veterans; and remains up-to-date on suicide prevention training materials and clinical tools, and utilizes as appropriate in clinical practice. - Making recommendations for referral of repeat attempters to a variety of appropriate programs with the Service Line. - Developing and presenting recommendation briefings to the Suicide Risk Reduction Committee for veterans with High Risk for Potential Suicide Patient Record Flags. - Maintaining awareness of suicide treatment options and making recommendations to the veteran's treatment team when new options become available. - Monitoring that all "High Risk for Potential Suicide" veterans have completed a Safety Plan/Suicide Risk Reduction Plan. Safety Plans are completed by the veteran in conjunction with his/her Principal Mental Health Provider, or an inpatient treatment team. The Safety Plan will include ways to get assistance if an immediate crisis should occur, and the veteran will be provided a copy of the safety plan. - When it is clinically determined that a veteran is no longer at "High Risk for Potential Suicide", the case manager collaborates with the Principal Mental Health Provider to oversee the transition of care to assure that appropriate behavioral health follow up is in place. - Completing issue briefs for suicides, utilizing data information and tracking systems for high risk patients, reporting data and metrics related to suicide and suicide behavior within the service line and organization - Providing staff and patient education regarding suicide prevention strategies and interventions - Conducting community outreach events and coordinating suicide prevention activities with non-VA community partners and stakeholders

Work Schedule: Monday - Friday 8:00am - 4:30pm

QUALIFICATIONS REQUIRED:
BASIC Requirements : Must be a US Citizen.

Education: Have a MasterÂ's degree in social work from a school of social work accredited by the Council on Social Work Education. Licensure: Be licensed or certified at the MasterÂ's level to independently practice social work in a state. English Language

Proficiency :
Social Workers must be proficient in spoken and written English.

GRADE DETERMINATIONS:
SALARY IS TO BE DETERMINED BY THE PROFESSIONAL STANDARDS BOARD. In addition to the basic requirements for employment, the following criteria must be met to

determine grade level of the position:
GS-11: In addition to meeting all basic requirements, 1 year of professional social work experience, equivalent to the next lower grade level, under qualified social work supervision. This experience must have been in a clinical setting and must have demonstrated the potential to perform advanced assignments independently.

NON-QUALIFYING EXPERIENCE :
Clinical social work experience obtained prior to completion of the requirements for a MasterÂ's Degree in Social Work does not satisfy this requirement. NOTE: A clinical setting is a medical center, clinic (medical or psychiatric), a residential treatment center, or any other type of facility where social work participates in collaborative treatment and is identified with the medical profession. or In addition to meeting basic requirements, a doctoral degree in social work from a school of social work. A doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. FOREIGN EDUCATION REQUIREMENTS: To receive credit for education completed outside the United States, you must show proof that the education has been submitted to a private organization that specialized in interpretation of foreign educational credentials and such education has been deemed at least equivalent to that gained in conventional U.S. education programs. Qualifications: Applicants must meet the minimum qualifications as stated in VA Handbook 5005/25, Part II; Appendix F7 dated 4/15/2002, available at http://vaww1.va.gov/ohrm/HRLibrary/Dir-Policy.htm , Qualification Standard Social Worker GS-185.

If you are using foreign education to meet qualification requirements, you must send a Certificate of Foreign Equivalency with your transcript in order to receive credit for that education.
