Description
This Behavioral Health RN coordinates and assures quality care processes for both acute and chronic psychiatric patients. This service will support physician practice, maximize outpatient productivity, insure quality care across the care continuum, and reduce inpatient costs. The ideal candidate will have extensive Behavioral Health experience.
Clinical responsibilities:
- Provide phone consultation to psyc! hiatric patients. Consultations to include; triage, returning ! physician calls, crisis calls of a medical nature, and patient outreach calls when necessary. Content of calls will vary with multiple questions about medications, side effects, symptoms, dosages, contraindications and refills. In selected cases this service may be provided on a face to face basis i.e. in clinic visit
- Intensive management of an identified and referred high risk (inpatient history) population of patients. These services to include; psychosocial assessment, development of a care plan, interventions designed to achieve care plan goals, education, monitoring, and linkages to GHC and community resources when necessary. Provide in clinic support visits and off site visits when necessary and warranted. Enter and maintain care plans, services, notes and sick day plans in Caretracker
- Systematic, planned care approaches for major DSM V diagnostic groups of individuals. To include Depression Roadmap, Bipolar Groups.
- Coordinate care process f! or patients to assure quality. To include; communications with BHS, GHC or community providers all information that is in support of the care plan, communication with patient and family all matters impacting the care plan, facilitate application process for patient assistance programs, coordinate required labs, schedule and attend appropriate care conferences, as well as Clinic and Department team meetings
- Facilitate medication refill authorizations in conjunction with staff and psychiatrists.
- Provide backup coverage for other Psychiatric RN's and HLN's when they are absent from the clinic.
Education
Minimum Education Requirements
Nursing Degree from an accredited program.
Preferred Education
Bachelor of Science in Nursing (BSN).
Knowledge/Skills
Minimum Knowledge/Skills Requirements
Communication, problem-solving, leadership and decision-making skills.
Preferred Knowledge/Sk! ills
Knowledge of care management in an inpatient or outpatient beh! avioral health setting.
Experience
Minimum Experience Requirements
1 year
Preferred Experience
Ambulatory care nursing experience. Primary nursing or case-management experience. 2 years in a behavioral health or psychiatric environment.
License/Certifications
Minimum License/Certifications Requirements
At time of hire Current Washington State RN License.
Preferred License/Certifications
Psychiatric Nursing Certification/education
