Work Schedule: Monday - Friday, 8:00am to 4:30pm, although e vening and weekend tours may be required for outreach events.

Duties:
The principle role of the Suicide Prevention Case Manager is to coordinate and monitor services for Veterans designated as "High Risk for potenial suicide," and act as the transitional coordinator of care for these patients. The incumbent is responsible for coordinating the care of high risk Veterans throughout the medical center, including Community Based Outpatient Clinics (CBOC) and community agencies. The incumbent will collaborate with inpatient treatment providers to support appropriate discharge planning following recommendations for treatment of patients at high risk for suicide; assuring all Veterans assigned to the facility "High Risk for Potential Suicide" list are followed in accordance with regulatory guidelines; includin! g face to face/telephone contact, tracking no-show appointments, intiti! ating emergency interventions as appropriate, etc. The incumbent will provide comprehensive evidence-based psychotherapeutic interventions including individual and group psychotherapy. The incumbent will provide consultation to health care providers about assessment findings and treatment plans for veterans to ensure care is coordinated, collaborative, and integrated within the medical center. The incumbent will serve as a resource for other professional staff throughout the facility. The incumbent will coordinate and collaborate with legal representatives, court officials, and community providers. The incumbent will develop, implement, and document the psycotherapeutic treatment plan for an assigned caseload of veterans, including prompt completion of clinical reminders. The incumbent will conduct routine risk screens. The incumbent will conduct comprehensive assessments and collaborative safety plans when clinically indicated. The incumbent will conduct follow-up appointm! ents for all veterans upon initiation on the "High Risk for Potential Suicide List" in compliance with VA directives. The incumbent will follow-up with veterans who fail to attend appointments per local procedures. The incumbent will consult with the Suicide Prevention Coordinator, a member of the Suicide Risk Reduction Committee, or a Mental Health Service supervisor as needed to ensure the safety and welfare all veterans. The incumbent will remain updated on suicide prevention training materials and clinical tools, and utilizes as appropriate in clinical practice. The incumbent will independently respond to inpatient consults. The incumbent will provide consultation and supervision to social work trainees and unlicensed staff as indicated. The incumbent will support and participate in program develop efforts. The incumbent will assume complete professional responsibility for clinical assessment findings, veteran care decisions, and documentation. The incumbent may be requ! ired to perform community outreach efforts.

BASIC

REQUIREMENTS:
The
following are requirements for appointment as a social worker in the Veterans

Health Administration (VHA):
a.
Citizenship: Be a citizen of the United States.

b.
Education: Have a MasterÂ's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the masterÂ's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. COPY OF TRANSCRIPTS MUST BE SUBMITTED WITH YOUR APPLICATION PACKET.
d. Physical
Requirements: All applicants must meet the physical standards for the position. A physical examination prior to employment may be required. The physical demands of this position involve primarily walking, talking, driving, use of the telephone, etc. Strenuous physical activity is not required although the incumbent is expected to withstand pressure and stress as part of their day-to-day activities.

Proficiency: Social workers must be proficient in spoken and written English.

REQUIREMENTS:
GS-12 Social Worker:
Experience, Education and Licensure. Individuals assigned as a social worker for a specific program or as a program coordinator must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or! other post-masterÂ's degree training from a nationally recognized prof! essional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. The program coordinator manages the daily operation of the program, develop policies and procedures for program operation and prepare reports and statistics for facility, VISN, and national use. They also may be responsible for program's budget. PROOF OF AND COPY OF ADVANCED PRACTICE LICENSE MUST BE PROVIDED.
In addition, the candidate must demonstrate the professional KSAs in below.
1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness. Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority for services and provide specialized treatment s! ervices.
2. Advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans with polytraumatic injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, etc.
3. Advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on empirical findings.
4. Ability to provide subject matter consultation to colleagues and students on the psychosocial treatment of pat! ients treated in the specialty area, rendering professional opinions ba! sed on experience and expertise and role modeling effective social work practice skills. Ability to teach and mentor staff and students in the specialty area of practice and to provide supervision for licensure or for specialty certifications.
5. Ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program.
