DUTIES:
The Mental Health Service provides in-patient care for Veterans with acute and chronic health conditions, with age ranges from young adult to geriatric and diverse in culture and religious/gender orientation. Applicants must demonstrate the ability to serve as a responsible member of the nursing care team and interact with patients, family members, professionals and other supportive personnel involved in the delivery of patient care. The LPN is an integral member of the Acute Inpatient Psychiatry Team, responsible to the Nurse Manager while also working under the direct supervision of a Registered Nurse for provision of care to patients on the inpatient mental health unit.
The LPN:!
demonstrates independent technical judgment with minimal supervisi! on, as well as maintains a comprehensive and thorough working knowledge of the most complex and difficult practices and procedures
possesses the ability to observe more subtle physical and emotional changes in patients and demonstrates taking appropriate action(s) based on those observations
accurately and thoroughly documents observations, care delivered and the patient's response to care that could require immediate modification of the patient's care plan
seeks assistance and guidance by informing RN or MD of changes in patient's condition requiring higher-level intervention
possesses the ability to proactively assist the RN or MD in addressing patient's needs for medication/treatments; accurately administer prescribed medications/treatments according to established policies/procedures; and appropriately records administration and patient's response to medications/treatments received
demonstrates the ability to use the computer! ized documentation systems (BCMA, CPRS, VISTA) at VAPSHCS
QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Graduate of practical or vocational nursing approved by the appropriate State accrediting agency and/or the National League for Nursing at the time the program was completed by the applicant. Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
Licensure: Full, active current and unrestricted LPN licensure in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or in the District of Columbia.
QUALIF! ICATIONS/GRADE DETERMINATIONS:
Applicants must meet the minimum! qualifications as stated in VA Handbook 5005, Part II, Appendix G13, dated 4/10/2003. In addition to the basic requirements applicants must meet the specialized experience requirements as follows:
GS 4: Six months of qualifying experience as an LPN or LVN; or Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or Graduation from an approved school of at least 24 months duration.
GS-5: Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent. Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Individuals a! t this grade level are expected to have a broad working knowledge of practical nursing procedures. However, completion of more complex practices or procedures may be subject to closer higher-level review.
GS-6: Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN. Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for appropriateness and conformity with established policies/procedures. Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to ! support duties for these more complex, specialized tests/procedures.

Physical Requirements:
This position involves considerable amount of standing (up to 2 hours), walking (up to 6 hours), stooping, bending, reaching, lifting, and turning. It requires restraining confused clients, repositioning patients, assisting with transfers from wheelchairs/stretchers and transporting them to other departments or facilities. Mechanical devices are available and are required to be used for heavy or difficult to lift clients. The work may involve pushing or lifting in excess of 45 pounds and going up and down flights of stairs. The position requires visual (corrected to 20/30 or better with color discrimination) and auditory acuity at a level to provide safe and effective nursing care. The position requires the nurse to work for periods of time with arms above shoulder level or with! neck in a fully flexed or extended position. The nurse may occasionall! y be exposed to patients who are combative secondary to delirium, dementia or psychiatric disorders. The nurse many be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The position also requires ability to work on computers for extended periods of time.
