There are five organizational divisions within ADSA. This position is within the Division of Developmental Disabilities; Fircrest Residential Habilitation Center (RHC) located in Shoreline, WA, just north of Seattle. Fircrest RHC is located on a beautiful 80+ acre campus. Fircrest offers free parking, a walking path around campus, and a fitness room available to staff. Our campus is near several restau! rants, shopping locations including Northgate Mall and the Met! ro bus-line.
Fircrest is currently recruiting for a P ermanent Clinical Pharmacist . This position is PART - TIME ; 20 hours per week , with Saturdays and Sundays as regularly scheduled days off . Work hours will occur between 8:00am and 5:00pm and may vary. THIS POSITION WILL REMAIN OPEN UNTIL AN ADEQUATE CANDIDATE POOL IS AVAILABLE. Duties:
SCOPE OF WORK Performs advanced comprehensive consultative and distributive pharmaceutical services to clients with complex medical, developmental, and neurological disorders residing at Fircrest Residential Habilitation Center (RHC) located in Shoreline, Washington.
Provides comprehensive clinical services to the clie! nts and staff of Fircrest School and assists the Pharmacy Director with coordination of the provision of clinical pharmacy services.
ASSIGNED DUTIES AND RESPONSIBILITIES CLINICAL: Provides comprehensive, clinically-oriented distributive pharmacy services to the clients. Perform related functions under the general direction of the Pharmacy Director. Works as an active member of the interdisciplinary team to ensure provisions of safe, efficacious, cost-effective, pharmacotherapy based on application of the most current principles of pharmacology, pharmacokinetics, and therapeutics. Review, evaluate and monitor individual client past and present medical regimens, medication usage, drug therapy profess and related clinical date: drug, dose, dosage form, route of administration, administration times, drug-drug and drug-disease interactions, allergies, symptoms, and adverse reaction potential; assess clients; formulate and communicate recommendations for analysis,! monitoring and evaluation; facilitate implementation of recommended ch! anges in individual clients pharmacotherapy regimens when appropriate. Provide pharmacotherapeutic information and client data during monthly SNF reviews, 90-day medication reviews, neurology, psychiatry and other clinics and team meetings. Coordinate and monitor all clients controlled on clozapine and serves as a communication liaison between the consulting psychiatrist and interdisciplinary team. Also serves as coordinator for neurology/psychiatry database to allow evaluation for use of antiepileptic and psychiatric medications, and monitor trends in use of these drugs. Participate in clinical pharmacy related activities including quality assurance, drug utilization reviews, research and publication; monitor and report adverse effects; evaluate and report potential and actual medication errors; critically evaluate drug related medical literature. Communicate effectively with other professional and paraprofessional staff concerning appropriate drug utilization. Provide acc! urate drug information regarding therapeutic, interactions, and adverse effects to medical, nursing and other staff, parents/guardians or other appropriate persons. Precept and mentor pharmacy students on rotation. Participate and support the Pharmacy and Therapeutics Committee (P&T), Pharmacy Services Committee (PSC). Develop, review and maintain policies and procedures related to pharmacy services at Fircrest. DISTRIBUTIVE: Utilize and maintain pharmacy electronic patient profiles; enter medication orders, record fills and credits, and document as required by law. Supervise and coordinate daily technician duties. Perform accurate final checks. Maintain continual pharmacy operations and provide efficient pharmacist support. Monitor and report potential and actual medication errors and adverse effects.
Qualifications:
POSITION SPECIFIC QUALIFICATIONS
LEGAL REQUIREMENT: A valid Washington State License to practice pharmacy or eligibility t! o obtain licensure prior to employment. Permanent appointment will be c! ontingent upon obtaining a valid Washington State License to practice pharmacy within 90 days following appointment. EDUCATION REQUIREMENT: Graduation from an accredited college of pharmacy with a Doctor of Pharmacy (Pharm.D.) degree or Master of Science (M.S.) degree with clinical emphasis
OR
Bachelor of Science degree in Pharmacy with Clinical Emphasis, a completed Residency Program in Clinical Pharmacy AND TWO years of clinical consultative pharmacy experience
OR Bachelor of Science degree in Pharmacy AND the equivalent of FIVE years of clinical pharmacy consultative experience.

Maintain all required certifi! cations and other mandatory training including but not limited to CPR, First Aid, and Bloodborne Pathogens.
